EEOP Business Funded Internship

Work Site Provider Donation Agreement
The Evening Education Options Program (EEOP) and the Work Site listed below agree to provide a funded work site position for the following student:


Work Site Provider Information











The Work Site Provider agrees to allow the student listed above in a designated position equal to $8.00 per hour for a minimum of 15 hours per week, not to exceed 20 hours per week. Duration of work site experience shall be 15 weeks.
